Genetic & Epigenetic Testing Informed Consent

1. Purpose of Testing

2. What Is Tested
Genetic testing analyzes your DNA across pathways relevant to nutrition, fitness, sleep, hormones, methylation, detoxification, and longevity. 3. Voluntary Participation
Your participation is entirely voluntary. You may withdraw at any time before your samples are processed. Once samples are processed and results generated, prior results cannot be undone, but you may request deletion of your data subject to our retention policies. 4. GINA Protections
The Genetic Information Nondiscrimination Act of 2008 (GINA) generally prohibits health insurers and employers from discriminating against you based on your genetic information. 5. Use of Your Sample

6. Data Storage and Security

7. Risks and Limitations
Genetic information is probabilistic, not deterministic. A risk variant does not guarantee a condition; absence of a variant does not guarantee its absence. 8. Family Implications
