Member Service Agreement

1. Parties and Acceptance

2. Services Provided
VitalityX provides a personalized health-optimization service that may include genetic interpretation, biomarker analysis, lifestyle coaching, custom supplementation, training guidance, and access to the member portal. 3. Membership Tiers
VitalityX offers multiple membership tiers (Ignite, Elevate, and Transcend), each providing different levels of access to Services. The features, pricing, and terms of each tier are described on our website and may be updated from time to time.
4. Auto-Renewal and Click-to-Cancel
Your membership renews automatically on a monthly basis at the rate disclosed at checkout, unless you cancel before the renewal date. You may cancel at any time from your member portal in just a few clicks (no phone calls required), in compliance with the FTC Click-to-Cancel rule and California Automatic Renewal Law. 5. $500 Full Decode Membership Credit
If you purchased VitalityX Decode or Full Decode and upgrade to any membership tier within sixty (60) days of your final results being delivered, $500 is automatically applied to your membership. The credit is not redeemable for cash and applies only to membership signups. The window starts when your final results are delivered, not at the time of purchase.
6. Refund Policy
Membership fees are refundable within 14 days of your first payment if you have not yet redeemed coaching sessions or other consumable services. 7. Medical Disclaimer
VitalityX provides wellness, fitness, and lifestyle services. We are not a medical practice; our coaches are not licensed physicians; and nothing on the platform constitutes the practice of medicine, the diagnosis of any condition, or a prescription. 8. Use of Your Data
Use and disclosure of your personal and health information is governed by our Notice of Privacy Practices and Privacy Policy. 9. Term and Termination

10. Dispute Resolution

11. Governing Law and Venue
